Die Versorgung von Frakturen langer Röhrenknochen beim Polytrauma - Validität der aktuellen Literatur und Konsequenzen für die Erstellung von Leitlinien

The method of choice for fracture stabilization in polytrauma patients is still an unresolved problem. Our analysis was intended to evaluate the quality of the specific literature presently available and, basing on the results, to give recommendations for the method of choice for the fracture fixation in polytrauma. Based on the results of our analysis, using the "USP"-protocol, the evidence of the literature presently available is not sufficient to draw conclusions for guidelines. Calculating the pooled Odds Ratio for the development of postoperative complications, there is a slightly better outcome for "Early Total Care" (ETC) when compared to a definitive fracture fixation later than 24 h after trauma. Immediate, primary fracture fixation with plate osteosynthesis gives the smallest Odds Ratio. The validity of clinical trias using this method, however, has to be improved. Undoubtedly there is a dramatic need for further research, as, according to our results, the presently available literature gives no hard criteria for the choice of the optimal method of fracture treatment in polytrauma patients.
