Ergebnisse nach stentgestützter Versorgung traumatischer thorakaler Aortenrupturen

Purpose: To present the results of endovascular repair of acute traumatic descending aortic transection. Methods: Between 1999 and 2005, 27 patients (20 men; mean age 37 years, range 17 - 83) were treated emergently for aortic transections. The devices used included 21 TAG Excluder stent-grafts, 4 Talent stent-grafts, 5 Cook stent-grafts and 1 Optimed stent-graft. Results: No patient required conversion to an open transthoracic operation. No patient developed permanent neurological deficit after endovascular treatment. Four patients developed a primary endoleak after the endovascular treatment (3 type I endoleaks and 1 type II endoleak). One patient died because of the type I endoleak. Three patients died secondary to injuries unrelated to the aortic repair. Conclusions: The treatment of acute traumatic descending aortic transection with an endovascular approach is feasible and safe and may offer the best means of therapy. Mortality and the risk of neurological deficit are low compared with open operations.
