Nutzerbewertung von Diagnostik am Beispiel Kernspintomographie: eine Befragung von Patienten und ärztlichem Personal
Objective 1. To research patients and physicians willingness of taking part in a study like the FFP (Fix-Flex-Protocol), in which DT (diagnostic testing) can be assessed on the level of therapeutic effectiveness. 2. To describe patients and physicians expectations towards diagnostic testing and MRI (magnetic resonance imaging) 3. To measure the influence of information on the willingness of taking part in the FFP. Methods Patients in hospitals in Ulm, Dresden and Berlin (Germany) and Brixen (Italy) with injury or trouble of shoulder, knee or ankle joints were randomized to one of two questionnaire versions: one version with extra information about MRI and one without. Orthopedists got the version without extra information. Results were compared by using Chi-Square-Test. Results 860 patient versions have been sent to the hospitals and 497 returned. 14 were excluded. 51 physician versions were returned. Willingness for taking part in the FFP was 70 % in the patients group and 71 % in physicians group. Extra information had no significant effect (p = 0,173). 98 % of the asked thought that physicians and nurses know patient’s needs. 95 % expressed the opinion that patients should be asked before testing. 57 % of the physicians and 96 % of the patients believe that patients should have the right to demand a diagnostic test. 43 % of the physicians and 88 % of the patients think the more DT means the better outcome. 52 % of the patients and 88 % of the physicians think that DT can lead to a wrong conclusion. 38 % of the patients and 86 % of the physicians agree to this in the case of MRI. 70 % of the patients and 98 % of the physicians think that clinical experience is more important than MRI. Conclusion High willingness of taking part shows that a clinical trial like the FFP seems to be possible. Patients and physicians differ in the assessment of diagnostic tests, MRI and clinical examination. The results generating hypotheses that seem worth researching further.
