AbstractThe outcome of patients with severe head trauma depends on the irreversible primary lesion and secondary brain damage caused by hypoxia, hypercarbia and hypotension. Therefore, it is necessary to provide adequate support in the field and in the trauma center to prevent improving secondary brain damage. Guidelines are given about the management for the rescue service and the primary care in hospital. This prospective study was undertaken analysing the status quo in the quality of care in these patients given by ambulance car and emergency physician or rescue helicopter and in the emergency department. Between 01.05.1999 and 15.08.2000 291 trauma patients were seen in the emergency room of the army hospital in Ulm (Bundeswehrkrankenhaus). Searching for patients with severe head injury, 61with Glasgow-Coma-Scale<9 and 64 with Abbreviated Injury Scale Scull ³4 were found. In both groups 38 patients were served by helicopter, the others by ambulance car. Procedures providing oxygenation and stable cardiovascular function as in the guidelines requested were found in mostly all helicopter served patients.
