AbstractChronic heart failure (CHF) leads to reduced physical performance and decreased life quality. This reduction results not only from the under-perfusion but also from pathological changes in the skeletal muscle. So, the treatment should include measures which can reverse or improve these changes. Physical exercise induces cellular mechanisms in the muscle in adaptation to training, among which the 70 kD heat shock protein (Hsp70) response plays an important role. This study was to investigate Hsp70 response in the skeletal muscle to strength endurance training in patients with CHF. 25 male, clinically stable patients with CHF (NYHA I-III) were enrolled in the study. A standard therapy program was applied to the controls (C) (n = 11), and an additional 12 week strength endurance training (65 % 1RM, ca. 45 min/unit, 3 units/w) was performed by the training group (T). Before and after the training, physical performance, leg strength, hemodynamic parameters as well as muscle biopsies (m. vastus lat.) were obtained. Hsp70 expression was quantitatively analysed at protein level with Western-Blot and at mRNA level with real-time PCR. There was no difference between both groups before training, and in C no change was found between before and after training with respect to all the obtained parameters. In T, physical performance increased from 125 to 150 Watts (P < 0.01), and leg strength from 175 kg to 260 kg (P < 0.01). Hsp70 expression at protein level did not change in both groups. In T, Hsp70 mRNA increased significantly higher than that of C (P < 0.05). In conclusion the applied strength endurance training led to significant improvement in performance and exercise tolerance without worsening hemodynamics and cardiac function, and has therefore a beneficial effect on CHF. This effect is accompanied by an increased Hsp70 expression in the skeletal muscle, implying that Hsp70 may play a role in the muscular adaptation to exercise in patients with chronic heart failure.dc.description.abstract
